Golden Oak Dental Premium Plan

THE GOLDEN OAK DENTAL PREMIUM PLAN IS A YEARLY PLAN FOR OUR PATIENTS WITHOUT TRADITIONAL DENTAL INSURANCE.

Unlike traditional dental insurance there are no yearly maximums, deductibles or waiting periods.

Included in our Premium Plan 

ADULTS:
2 recall exams
2 regular prophylaxis
1 set of x-rays (bitewing or panoramic)
2 oral cancer screenings

CHILDREN: (14 AND YOUNGER)
2 regular prophylaxis
1 set of x-rays (bitewing or panoramic)
2 fluoride treatments

This plan automatically renews every 12 months
A 15% discount will be provided for any additional treatment
Invisalign and Whitening excluded from discount

Premium Plan Costs:

• 1 MEMBER = $325.00
• 2 MEMBERS = $600.00
• 3 MEMBERS = $800.00

EACH ADDITONAL FAMILY MEMBER = $200.00
based on 2017 fees
